Hospice@Home provides End of life Care for patients who wish to remain at Home ( adults over the age of 18) we provide day & night respite for family members too. Our Family Bereavement service provides support on a one to one basis or with weekly drop in cafes either at Berwick or Alnwick. We also offer Dementia support service provides advice, activities and twice monthly cafes for anyone diagnosed with Dementia.
Hospice at home provides high quality, holistic care at the End of Life, day or night to adults over the age of 18, by qualified Nurses & Hospice support workers. A qualified health professional like your GP, District Nurse or MacMillian Nurse can refer you into our service.
The Family Bereavement service provide support for family members either pre or post bereavement over the age of 18. Our family services coordinator offers 121 active listening or group sessions within a drop in cafe at Berwick or Alnwick, anyone can can self refer.
Our Specialist Dementia Nurse can offer advice & support to carers. The dementia support service includes drop in cafes & seated exercise classes at both Berwick and Alnwick. And walking football at the the leisure centres at Alnwick & Berwick. Anyone can self refer.

Face to face,in a persons own home or at the Alnwick or Berwick well being centres ,by telephone or in group sessions.
